2011 Flexible Spending Account

Expenditure Deadline - March 15, 2012

If you have money left over in your 2011 FSA account, you have a Grace Period until March 15, 2012 to incur expenses using your remaining 2011 funds. If you enrolled in FSA for the 2012 Calendar Year and use your debit card, any remaining balance from your 2011 account will be used before accessing your 2012 funds. Stanley, Hunt, Dupree, and Rhine (SHDR) must receive paper claims by May 31, 2012 in order for you to be reimbursed from your 2011 account.

This form is used for submitting manual claims.
