As previously communicated in December 2016, you will receive a Form 1095-C from UNM mailed to you at your current address of record. The Form 1095-C is used on your tax return to verify that you, your spouse, and any dependents had coverage for each month you were enrolled in UNM’s medical plan in 2016.
The IRS has extended the deadline from January 31 to March 2, 2017, for employers to issue 1095-C Forms for 2016 to their employees. UNM will mail the required 1095-C Form to you on or before March 2, 2017.
While the information on the Form 1095-C may assist you in preparing your tax returns, it is not necessary to wait to receive this form from UNM to file your individual income tax return. You should file your return as you normally would. If you have already filed your 2016 income tax return, the IRS will not require you to amend or re-file your return once you receive the Form 1095-C.
