To participate in the checkups, active, UNM benefits-eligible employees and their spouse/domestic partners must be actively enrolled in a UNM Medical Plan during the onsite preventive health checkups (July 8 - October 18, 2019) and continuously enrolled through the pay period(s) when the premium credit will be applied (late fall 2019). |
Yes, as long as you continue to meet the eligibility requirements listed above. You can receive a medical premium credit this year regardless of whether you participated last year. This year’s credit applies to your medical premiums for 2019-2020. We encourage you to participate in the onsite preventive health checkups and take advantage of the premium credit on a yearly basis. |
The preventive health checkups will include tests measuring your overall health. Tests include blood pressure, cholesterol, glucose, BMI, waist circumference, and more. Nurses will collect lab accurate bloodwork via a finger stick, which is processed onsite. Once you receive your results you will tele-consult with a board certified Nurse Practitioner. Catapult Health is the vendor providing health screenings. Catapult Health is a national Preventive Care Medical Practice specializing in worksite preventive checkups. Their nurses collect lab accurate bloodwork via a finger stick which is processed onsite and consulted upon by a board certified Nurse Practitioner. There is no out of pocket cost to you for the health checkup. Catapult will submit a claim for preventive services to your UNM Medical Plan TPA, who may send you an Explanation of Benefits. This preventive services claim will be fully covered by your UNM Medical Plan; there is no payment necessary on your part for the preventive health checkup. Health checkups will be scheduled at numerous locations on all campuses (including branch campuses). Visit the sign up page to find the most convenient location for your schedule. Additional sites may be added based on demand. |
Yes! All health checkups will be done by appointment only. You will not be able to walk-in without an appointment. Sign up to reserve your time. |
Yes. Each individual participating in the onsite preventive health checkups will need their own appointment. If both you and your spouse/domestic partner wish to participate, each of you will need your own appointment under your own name. When you visit the sign up page, click “Make a New Appointment” to create the appointment for your spouse/domestic partner. |
Your visit will take about 30 to 45 minutes. |
No, you do not need to use leave time for checkup appointments, including travel to and from the checkup site. In most instances, we would not expect the appointment and travel time to exceed 1 hour. As always, work with your supervisor to ensure work responsibilities are properly covered. |
If you already received a health checkup from your doctor or you wish to visit your primary care provider to receive testing between October 20, 2018 and October 18, 2019, you can submit your results using the Primary Care Provider Form. Your doctor will fill out the form and YOU must submit the completed PCP form between June 3, 2019 and October 18, 2019. THE FORM MUST BE POSTMARKED NO LATER THAN 5:00 pm (MST) on October 18, 2019, in order to qualify for the premium credit. If you would prefer to submit your Primary Care Provider Form to Catapult Health via secure email, contact their customer support team at 877-234-1840 or support@catapulthealth.com. You will receive a confirmation email once your form has been received and processed. If you do not receive this confirmation e-mail within two weeks of submission, contact Catapult Health at 877-234-1840 or support@catapulthealth.com. |
For 2019, the credit is $200 for eligible employees and $100 for eligible spouse/domestic partners. After you complete your preventive health checkup, you will receive a reduction on the medical premium in your paycheck. Depending on your situation, the credit may be spread across multiple paychecks until it is fully used up. |
After you complete your health checkup, a credit should appear in your paycheck in late fall of 2019. The credit may be spread over several pay periods. |
