The University of New Mexico, as a hybrid covered entity under 42 C.F.R. Part 164.504 hereby designates UNM Medical Plan as health care components for purposes of complying with the Health Insurance Portability and Accountability Act of 1996.

As an employer with a self-insured medical plan, UNM is required to inform benefits-eligible employees about certain rights and protections provided under Federal Law. According to new Federal regulations, as an employee eligible for health coverage provided through The University of New Mexico (UNM), you are entitled to receive a Form 1095-C for the 2025 tax year, which serves as proof of the coverage offered to you. This Form was previously mailed only to individuals who had not eConsented to receive it electronically; however, due to new regulations aimed at reducing paperwork and administrative burden, UNM is no longer required to automatically send paper copies via mail. To request a copy of your 2025 Form 1095-C, you can:

Forms will be provided by January 31 or within 30 days of request, whichever is later.
Note: You do not need to wait for this form to file your tax return, but you should keep it for your records.
