Employee’s Preparation Checklist for Performance Planning (PLAN)

Use this Employee Preparation Checklist to guide you through the planning process: 

_______1. Review your college or department's mission and goal statements. This will help you ensure that performance goals will be supportive of those of the higher-level organization.  

 

_______ 2. Review your job description.

   

_______ 3. Identify “SMART” goals and/or duties on which you may be evaluated and discuss these with you manager. Write these in Section 2.  

 

_______ 4. Discuss and identify with your manager behaviors that should be demonstrated in support of the University values. Refer to the examples of behavior listed in Section 3 of the form.

     

_______ 5. Discuss and identify with your manager any required and job specific training you may need. Write this in Section 4.
