UNM offers one vision plan through VSP to active benefits eligible employees and it is 100% employee paid. Participant plans offer low copays for eye exams, and discounts and allowances for contacts, lenses and frames plus more!
Enrollees receiving service from providers participating with VSP (in-network) will have no claims to file and the greatest cost savings. However, enrollees still have access to non-participating providers (out-of-network) as needed.
First, verify your eligibility.
Next, you may take advantage of these three enrollment opportunities:
Finally, if you are enrolling dependents, be prepared to provide “proof documentation”. Some Examples of Dependent Proof Documents are available here. Deadlines for submission of these documents vary in accordance with the enrollment opportunities listed above.
Note: If you separate from employment, termination of coverage will occur; however, you may elect to continue coverage in accordance with COBRA.
Make an appointment with your chosen doctor’s office, inform them you are a VSP member, and provide your UNM Banner ID number (not your social security number). VSP does not issue membership cards.
