Long-Term Care

LTC Basics

For unexpected events, as well as the aging process, The University of New Mexico (UNM) offers Long-Term Care (LTC) insurance through Unum.

LTC insurance covers some or all of the cost of custodial care of individuals who are no longer capable of caring for themselves. This type of care covers those affected by accidents, injury, or illness, and it is not restricted to the elderly.

  • Unum Plan Highlights, Premium Rates, and Enrollment Information
    • NOTE: Premiums are based on your LTC enrollment participation date
    • Fax applications to Unum at 207-541-7606 for processing
  • Coming Soon! Full-time Employee Certificate of Coverage through June 30, 2018
  • Coming Soon! Part-time Employee Certificate of Coverage through June 30, 2018
  • Coming Soon! July 1, 2017 Certificate of Coverage(full-time and part-time employees)

Eligibility and Coverage

Benefits-eligible full-time and part-time employees (excluding temporary adjuncts and temporary staff) are eligible to apply for Long-Term Care insurance through Unum. If you apply for LTC coverage within 60 calendar days from your date of hire, you are offered guaranteed coverage. Outside of the initial 60-day enrollment period (60 calendar days), eligible employees may apply for LTC coverage, subject to Unum underwriting approval.

Coverage for spouses, domestic partners, parents, and grandparents may also be purchased. Coverage is subject to Unum underwriting approval.

You may apply for LTC insurance at any time as a benefits-eligible employee. Enrollment in LTC insurance is not limited or subject to designated enrollment periods. Enrollment in LTC insurance outside of the initial 60-day enrollment period (60 calendar days) requires Unum underwriting approval.

To obtain underwriting approval, in addition to a standard application, you must complete Unum’s Evidence of Insurability Medical Questionnaire.

Cost of Coverage

Cost for LTC coverage is based on your age at time of enrollment. Therefore, the younger you are when you purchase LTC insurance, the lower your cost. To determine your LTC cost, use the Unum cost calculator!

To make changes to or cancel your LTC benefits:

Employees enrolled in LTC insurance have the option to make changes to their existing LTC coverage – to cancel, reduce, or increase existing coverage anytime (as applicable). Coverage for increases will be based on your underwriting approval date. Complete the Unum LTC change form to request a change or cancellation of your LTC coverage.

Submit completed Unum LTC Change Form to Unum

Prior to making changes to your existing coverage visit www.unuminfo.com/unm to learn more about Long-Term Care coverage and the financial security it may offer.

If you cancel any or all of your Long-Term Care coverage, you may re-enroll at a later date. However, your application for coverage will go through medical underwriting with Unum to determine eligibility. Deductions will commence only upon coverage approval from Unum.

Continuation of Coverage

After you separate from employment at UNM or retire, you may elect to continue group LTC coverage offered through Unum at the same cost. You must elect to continue coverage within 90 days from your separation or retirement date from UNM.

To request continuation of coverage, please contact the UNM Benefits office at 505-277-MyHR (6947) prior to the 90-day deadline.
